On Arrival At Work Each Day
On arrival at work each day you must complete a health declaration which states that you do not have any existing health conditions prior to starting work on that day and that you do not have a:
• fever
• tiredness
• diarrhea
• conjunctivitis
• headache
• loss of taste or smell
• a rash on skin, or discolouration of fingers or toes
• difficulty breathing or shortness of breath
• chest pain or pressure, or
• loss of speech or movement
and that you have not taken any prescription medication, illicit drugs or alcohol prior to starting work.
The Health Declaration must be completed each day prior to starting work.
